Hospital Discharge Service

When being discharged from hospital, you will be offered a referral to your pharmacy.

The purpose of the referral is to ensure that you are aware of any changes to your medication.

The pharmacist will:

• Advise you of any lifestyle and medication changes you might need to make
• Compare the medication you were previously taking with your updated prescription
• Ensure your GP has an up-to-date record of your changes
• Make sure any pending or uncollected prescriptions are updated to reflect the changes from your visit to hospital
• Help you understand that changes that have been implemented and how to get the most from your new medicines.
